In these sections, you are going to describe the roles of each individual in implementing this plan. What are their specific challenges, and how might they overcome that. Think about culture, finance, and collaboration.
THE ASSIGNMENT
You are the leadership team of a hospital that does not have a strong culture. The team members do not have a lot of structure, and they are not confident to speak up when they see something wrong. There is a separation between the physicians and the team members, and they do not always work well together.
The first part of the assignment (1-2 pages) is for you document the issues that could be associated with an organization like this. Remember to think about the Joint Commission, the Centers for Medicare and Medicaid, FMEA, root cause analysis, adverse events, sentinel events, and financial risk. Use your textbook and other assigned readings as appropriate for references. Next you will submit a 2-3 page Risk Management Plan. In this, I want you to research techniques used in the industry (TeamStepps is an example). Detail out all of the things that need to be implemented, who is involved, how does it roll out. HINT: A poor culture leads to safety issues, medical mistakes, etc.

The last piece will be a 1page conclusion in which you will tell how you plan, now fully implemented, has improved the issues that you initially documented.
The Group Process:
Ideally, you will work in groups of four on this project. The groups will be auto-assigned at random. Depending on the number of students in the class, some groups may have fewer members (3-4 students). Group members will work collaboratively on this assignment throughout the course. One final project will be submitted by a group member during Module 7. Each group member will also evaluate each member’s contribution to the project.
Each team must create and submit one APA-formatted document that includes:
- APA-formatted title page – All team members must be listed vertically in alphabetical order (by last name). The document must have a running head and all pages must be numbered. Please submit document in Times New Roman or Arial font, size 12, following guidelines for HSA writing. Use Level 1 heading for each section of the paper.
- Submit an APA-formatted References page (minimum of 3 peer reviewed references required).
Once assigned, your groups will complete of the following steps:
- No later than the end of Module 2, chose a leader, and have the group leader post in the Group Formation Discussion Forum the name of all the group members AND the name of the leader of the group.
- Post weekly to the individual group project discussion board of your progress and any questions/comments you might have.
- No later than Module 5, individual team members submit a draft of your part of the project to the group project discussion board so that the other members are able to see the progress.
- No later than the end of Module 6, individual team members should submit their final documents to the individual group discussion board.
- The group leader must submit the completed assignment no later than Midnight EST, Sunday Module 7. No other members should submit the project.
Final Team Project Grading
- EXCEPTION: If a determination is made that a team member contributed minimally to the completion of the project, that particular team member’s project grade may be reduced.
- Plagiarism: All documents will be checked for plagiarism. In the event plagiarism is determined to have occurred, all team members may receive a ZERO for the assignment.
- Teams are required to communicate through their team-specific discussion forums provided in the course. Participation in these forums will be considered when calculating points for professionalism and engagement.
- The team leader should save your final project as a Word document, named HSA4502_group#, paste the rubric to the last page of the paper, and upload it to the Final Group Project Dropbox in Module 7. Follow the Guidelines for HSA Assignments and read the rubric for grading details.
- Each group member will complete a Peer Review Form and upload it to the Group Peer Review Form Dropbox in Module 7. Group peer review forms will be considered when assigning grades.
